Backflow Prevention Assembly Test Report (119 kb • 07/04/2006) PDF Format

Leak Credit Request (74 kb • 07/04/2006) PDF Format

This form must be completed following the leak repair and returned to the District for a possible leak credit calculation. The request is then submitted for approval, depending upon the leak and adjustment amount. Customer will be notified as soon as possible regarding the status of the request regarding qualification and amount of adjustment.
