Backflow Assembly Testers List (169 kb) PDF Format

This is a list of certified testers provided for your convenience not as a recommendation by Water District #111. All testers must be certified by the Washington State Department of Health.

Backflow Prevention Assembly Test Report (119 kb) PDF Format

Backflow assembly devices are required to be tested annually. The attached form is a District approved test report however other test reports will be accepted. Please contact the office if you have questions, 253-631-3770.

Certificate of Water Availability Application (239 kb) PDF Format

Duplicate Billing Authorization (101 kb) PDF Format

Authorization for owners who would like a duplicate billing be sent to their renters.

Duplicate Billing Cancellation (101 kb) PDF Format

This form cancels the previous authorization for duplicate billing by a land owner.

Leak Credit Request (74 kb) PDF Format

This form must be completed following the leak repair and returned to the District for a possible leak credit calculation. The request is then submitted for approval, depending upon the leak and adjustment amount. Customer will be notified as soon as possible regarding the status of the request regarding qualification and amount of adjustment.
