Backflow Prevention Assembly Test Report (119 kb • July 4, 2006)

Leak Credit Request (74 kb • July 4, 2006)

This form must be completed following the leak repair and returned to the District for a possible leak credit calculation. The request is then submitted for approval, depending upon the leak and adjustment amount. Customer will be notified as soon as possible regarding the status of the request, qualification and amount of adjustment.

Public records are available for inspection and copying during the office hours of 8:00 AM to 4:30 PM, excluding holidays. Copies are 15 cents per page. Please complete request form to obtain record copies.
